Extended Care Program
VA Hudson Valley HCS has four (4) Community Living Centers with a total of 140 beds. Two are located at the Montrose campus and two at the Castle Point campus. Our purpose is to provide care with respect, dignity, autonomy and the individuality of each person. It is our goal to maintain a therapeutic environment that includes skilled nursing care, appropriate medical care, recreational and spiritual opportunities. Care is designed to assist each resident in maintaining integrity, self esteem, and identity through the encouragement and support of their self-care abilities. We provide an array of services to our veterans including: end of life care, respite, wound care, IV therapy and restorative rehabilitation, palliative care, skilled nursing care, maintenance care for those awaiting placement, and dementia care (wanderers with cognitive deficits).
We are committed by the vision of the Geriatrics and Extended Care Program to improving the quality of care as well as the quality of life for our veterans and their families.
Our mission is to serve the Health Care needs of America's Veteran's requiring extended care through a continuum inclusive of primary, specialized and rehabilitative care, community and social support services. We will embrace an environment which fosters mutual respect and trust.
